You have the right to:

  • Receive an accounting of disclosures made for purposes other than treatment, payment, or healthcare operations.

  • Request restrictions on certain uses or disclosures.

  • Request confidential communications via specific methods or locations.

  • File a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.
